We are located at 7 Joseph Street, Markham, ON L3P 2M8. If you have questions about this policy or about your information, please contact us at dcchiroclinic@gmail.com or (416) 371-9199.

Privacy laws that apply

As an Ontario health-care clinic, we handle personal health information in accordance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA). For website, administrative, and commercial activities, the federal Personal Information Protection and Electronic Documents Act (PIPEDA) may also apply.

Personal health information includes information that identifies you and relates to your physical or mental health, health history, assessment, treatment, care plan, appointments, billing, or health-card/insurance details. Personal information includes information that can identify you, such as your name, contact details, online form submissions, or website usage information.

Depending on how you interact with us, we may collect:

  • contact details such as your name, phone number, email address, and mailing address;
  • appointment details, intake information, health history, assessment findings, treatment notes, and related care records;
  • billing, payment, insurance, and direct-billing information where needed to process clinic services;
  • messages you send through our website, by email, by phone, through booking tools, or through social media;
  • website and device information such as IP address, browser type, pages viewed, referring pages, and cookie or analytics data.

How we use information

We use information only for reasonable clinic, care, administrative, legal, and website purposes, including to:

  • book, confirm, reschedule, and manage appointments;
  • provide assessment, treatment, follow-up, and continuity of care;
  • communicate with you about your inquiries, appointments, forms, or clinic services;
  • process billing, insurance, direct-billing, and payment matters;
  • maintain complete and accurate clinic records;
  • operate, secure, measure, and improve our website and patient experience;
  • meet professional, legal, regulatory, insurance, accounting, and reporting obligations.

Consent and your choices

We collect, use, and disclose information with your consent, except where PHIPA, PIPEDA, or another law permits or requires us to act without consent. Consent may be express, such as when you complete a form, or implied, such as when information is needed to provide the care or service you requested.

You may withdraw consent for certain optional uses, subject to legal, professional, or contractual limits. Withdrawing consent may affect our ability to provide some services. We will explain any practical consequences where relevant.

Disclosure and service providers

We do not sell personal information or personal health information. We may disclose information when needed for your care, clinic operations, billing, legal compliance, or with your consent. This may include regulated health professionals involved in your care, administrative staff, insurers or payment processors, laboratory or diagnostic providers, professional advisors, regulators, or other parties where permitted or required by law.

We also use trusted service providers to help operate the clinic and website, such as secure booking, form, hosting, email, analytics, payment, and records-management providers. These providers are expected to protect information and use it only for the services they provide to us. Some providers may store or process information outside Ontario or Canada, including in the United States, where it may be subject to the laws of that jurisdiction.

Website forms, booking tools, cookies, and analytics

Information submitted through website forms or online booking tools is used to respond to your request and support appointment administration. Please do not use website forms or email for urgent medical issues. If you are experiencing an emergency, call 911 or seek immediate medical care.

Our website may use cookies, analytics, pixels, or similar technologies to understand traffic, improve content, maintain security, and measure advertising performance. You can adjust cookie settings in your browser; some features may not work as expected if cookies are disabled.

Our website may link to third-party websites or services, including maps, social media platforms, review platforms, booking tools, or external resources. Their privacy practices are governed by their own policies.

Safeguards

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including access controls, staff confidentiality expectations, secure systems, and reasonable measures to protect records from unauthorized access, use, disclosure, copying, modification, loss, or theft.

Data retention

We keep personal information and personal health information only for as long as needed for the purposes described in this policy, unless a longer retention period is required or permitted by law, professional standards, insurance, accounting, or legitimate clinic needs. Retention periods vary depending on the type of information:

  • Clinical and personal health records: retained according to applicable health privacy laws and the requirements of the relevant professional college or regulator.
  • Website form submissions and inquiries: kept long enough to respond to your request, support appointment administration, document communications, and meet legal or clinic record-keeping needs. Information submitted through booking or form tools may be transferred into clinic systems when needed for follow-up or care.
  • Log, cookie, and analytics data: retained for a limited period needed to operate, secure, measure, and improve the website, unless a longer period is required to investigate security, fraud, or legal concerns.

Data deletion and disposal

When information is no longer needed for the purposes described in this policy, and no legal, professional, regulatory, insurance, accounting, or clinic need requires us to keep it, we securely destroy, delete, anonymize, or archive it in accordance with applicable requirements. You may request deletion of certain personal information, and we will consider the request where deletion is legally and professionally permitted.

Accessing or correcting your information

You may request access to your personal information or personal health information, ask us to correct information that is inaccurate or incomplete, or ask us to delete certain information where legally and professionally permitted. We will respond in accordance with PHIPA, PIPEDA, and applicable professional obligations. In limited cases, access, correction, or deletion may be restricted where the law permits or requires.

Questions or privacy concerns

If you have a privacy question, concern, access request, correction request, or deletion request, contact DC Chiropractic at dcchiroclinic@gmail.com, (416) 371-9199, or by mail at 7 Joseph Street, Markham, ON L3P 2M8.
